DATA MANAGEMENT
We provide customized support for clinical centres by collecting, analysing and interpreting health activity and clinical research data. This means we can find solutions and identify organizational practices that help them improve nursing and clinical practice on the ward.
REFERRAL PROJECTS
Our network-building initiatives aim to strengthen relationships and promote shared organizational practices between healthcare professionals in the community and those in hospitals, making it easier for clinicians (often GPs) to identify patients with chronic diseases and refer them to a consultant or a specialist team.
SUPPORT WITH CLINICAL FOLLOW-UP
These initiatives aim to promote regular and optimal monitoring of people with chronic diseases by the healthcare teams at clinical centres. We develop bespoke procedures and tools that are carefully modelled on the operational needs of the healthcare professionals concerned.
