The latter can observe the patient, assess their clinical condition, verify treatment adherence, and provide personalized guidance (patient-centered medicine<sup><a href=\"https:\/\/www.simenitalia.it\/patient-centered-medicine\/\">3<\/a><\/sup>), while maintaining a continuous relationship even from a distance. <\/p>\n<p><strong>Remote monitoring<\/strong> combines direct interaction modalities with telemonitoring devices \u2014 such as wearables, wearable sensors, and portable devices \u2014 which enable the collection of physiological parameters such as blood pressure, heart rate, blood glucose levels, and physical activity<sup><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10730976\/\">4<\/a><\/sup>.<\/p>\n<p>The collected data are transmitted to a dedicated digital platform and made immediately accessible to clinicians, integrating the gathered information and supporting a more comprehensive and timely assessment of the patient&#8217;s condition, also with a view to preventing clinical events.<\/p>\n<h4><strong>THE BENEFITS OF REMOTE MONITORING FOR PATIENTS AND HEALTHCARE PROFESSIONISTS<\/strong><\/h4>\n<p>The introduction of remote monitoring models can generate <strong>significant benefits<\/strong> for <strong>all stakeholders<\/strong> involved.<\/p>\n<p>For <strong>patients<\/strong>, the possibility of being monitored remotely through structured contacts reduces the need to travel to healthcare facilities, facilitating the daily management of therapy and\/or treatment and improving adherence. Furthermore, the presence of a constant point of reference helps strengthen confidence in the care pathway. <\/p>\n<p>For <strong>healthcare professionals<\/strong>, the availability of continuously collected clinical information \u2014 integrating objective data and observations derived from patient interactions \u2014 allows for a more comprehensive overview of disease progression and supports more timely clinical decisions<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/40988810\/\">2<\/a>.<\/p>\n<p>Furthermore, these models can contribute to reducing hospitalizations and the number of in-person visits, while simultaneously maintaining high levels of patient safety<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31314689\/\">1<\/a>.<\/p>\n<h4><strong>DIGITAL PLATFORMS AND ORGANIZATION OF HEALTHCARE SERVICES<\/strong><\/h4>\n<p><strong>Remote monitoring<\/strong> relies on <strong>dedicated digital platforms<\/strong> that integrate clinical data collection with interaction tools between patients and healthcare professionals<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31314689\/\">1<\/a>.<\/p>\n<p>These platforms represent a central tool for enabling the figures involved in the care pathway to actively follow the patient and to prevent and detect any critical issues<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31314689\/\">1<\/a>,<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/40988810\/\">2<\/a>.
